A Striker’s Certification Form MUST be completed and returned to your steward before you will be eligible for Relief Funds.
You can fill out this form online by going to the CWA LOCAL 9333 Online Striker Certification Form.
If assistance is needed during a prolonged strike, the CWA member can fill out the Striker’s Application For Assistance and turn it into your steward or picket line captain.
